Nurse Practitioner - Patient Aligned Care Team (PACT)
Incumbent provides outpatient care to eligible veterans with acute, chronic and/or special needs consonant with the Nurse Practitioner's licensure.
Incumbent works in the ambulatory care setting of the VA Montana Health Care System and supports the mission and vision of the VA Montana Health Care System.
The incumbent provides primary care, urgent care and/or compensation and pension exams for adult and elderly veterans.
Most patients present with chronic medical conditions, change in mental status, and/or functional decline.
Current BLS is required for this position.
Advanced Cardiac Life Support may be required related to specific procedures.
The ideal candidate would have a minimum of 2 years Nurse Practitioner experience providing chronic illness care.

Estimated Salary: $20 to $28 per hour based on qualifications.
